How to Appeal a Health Insurance Denial: A Step-by-Step Guide from a Patient Advocate

You did everything right. You saw your doctor, followed the treatment plan, and submitted the paperwork on time. Then the letter arrived: claim denied. Insurance companies deny claims for many reasons, and most of them have nothing to do with whether your treatment was medically necessary. The good news is that the majority of these reasons can be challenged and reversed — especially with the right support in your corner.


Step 1: Read the Denial Letter Carefully

Your Explanation of Benefits (EOB) or denial letter is a goldmine of information. Look for the specific reason code cited, the date of service in question, and the deadline for appeal. In most states, you have 30 to 180 days to file an internal appeal. Do not let that clock run out. Write down every date and deadline and keep all documents organized in one folder — paper or digital.


Step 2: Gather Your Medical Documentation

This is where the work really begins. Request your complete medical records from your doctor, including clinical notes, test results, and any letters of medical necessity. Your physician should write a detailed letter explaining why this treatment is not only appropriate but essential for your specific situation. Insurance companies respond to specificity — vague language gives them room to deny again.


Step 3: Write a Compelling Appeal Letter

Your appeal letter needs to be professional, specific, and grounded in both medical evidence and insurance policy language. Reference the exact policy language from your plan that supports coverage. Attach the letter of medical necessity from your doctor, relevant medical journal articles that support the treatment, and any clinical guidelines from professional medical organizations. State clearly what you are asking for and why you believe the denial was made in error.


Step 4: Know When to Escalate — and How

If your internal appeal is denied, you have the right to request an external review by an independent organization. Under the Affordable Care Act, you are entitled to this review at no cost to you. Additionally, you can file a complaint with your state's insurance commissioner. If your plan is employer-sponsored, the Department of Labor's Employee Benefits Security Administration can also intervene. Do not be intimidated — these are your legal rights.
